I understand that 5.3L 5w 40 is to be used, but when looking at an old service reciept 10w 40 had been used... will this of damaged the engine?
also... engine flushing.... any recomended additives?

DONT!
Engine flush causes more trouble then its worth
The correct oil & genuine / Purflux oil filters being used is much better for the engine then chucking flush bodging stuff in there.
