But the old knob has the white plastic sleeve stuck inside. How should I go about removing the white plastic sleeve inside the old knob? And what is a good after market brand for the leather gaiter? I like to maintain the stock look. eBay search came up with mk tuning, any opinions on that?

You will be hard pushed to get it out without breaking it, I'd stick it back on the leaver again and try to prise it out by making it catch on again, if not try any means possible but expect a trip to the dealers imo. As for gaters no idea don't usually bother with stuff like that myself.
